Admissions Application Form

Application Procedures:

 

To apply to Good Shepherd Catholic School, parents must submit the following required documents with a completed application:

 

  • A copy of the Birth Certificate
  • A copy of Immunization Records
  • A copy of the Baptismal certificate (if applicable)
  • A copy of First Communion certificate (if applicable)
  • A copy of the most recent Report Card (for grades K-8th)
  • A copy of current Test Scores
  • A nonrefundable application fee of $100 per child can be paid here: APPLICATION FEE


Once the application is submitted, a Family Interview with the Principal will be scheduled as well as an assessment appointment for the child.
This application does not assure final enrollment but provides information upon which a decision will be based.
